Getting Out and Staying Connected
Staying connected to the community can become harder without the right support, particularly for participants who need assistance with transport or accompaniment to activities. DMH Support Services provides community access on the Central Coast, supporting participants to get out for social activities, appointments and local events.
Support is arranged around each participant's interests and NDIS plan, rather than a fixed program everyone follows. This might mean a regular weekly outing, occasional support for specific events or something in between.
For participants wanting help with tasks closer to home as well, our in-home support page covers this separately. Call 0424 110 824 to talk through what community access might look like for you.

What Community Access Support Can Involve
Support is arranged following an individual assessment of a participant's interests and goals, and adjusted as those interests change over time. This can include:
Accompaniment to social activities, groups or events. Activities are chosen based on a participant's own interests.
Transport assistance, including wheelchair accessible transport. This helps participants get to and from appointments and activities.
Support attending appointments, such as medical or allied health visits. A support worker can accompany a participant throughout.
Support participating in group activities or programs. The level of involvement is set by the participant.
Setting up community access support starts with a conversation about the kinds of activities, appointments or events a participant would like support to attend. From there, a support arrangement is developed following an individual assessment of the participant's interests, NDIS plan and any specific access requirements, such as wheelchair accessible transport. Participants are asked to share relevant details from their plan so that support can be matched to approved funding categories.
A support worker is introduced ahead of the first outing, so participants know who to expect. Arrangements can be adjusted over time as a participant's interests or circumstances change.
