Support Built Around Individual Goals
Finding disability support that actually fits a person's own goals and daily routine isn't always straightforward, particularly when providers offer a standard package rather than something tailored. DMH Support Services provides disability support on the Central Coast, working with each participant to build support around their own NDIS plan.
This can include help with daily tasks, personal care or working toward specific goals identified in a participant's plan. Support is arranged with input from the participant and, where relevant, their family or support coordinator.
For participants wanting extra support with everyday tasks specifically, our in-home and daily living support page covers this in more detail. Call 0424 110 824 to talk through what support might suit your situation.

What Disability Support Can Include
Disability support is arranged following an individual assessment of each participant's plan and goals, rather than a fixed package applied to everyone. This can include:
Assistance with personal care and daily tasks. Support is arranged to fit each participant's own routine.
Support working toward goals identified in an NDIS plan. This might include skill-building activities relevant to a specific goal.
Coordination alongside other supports and therapies. This can include our therapy services, where relevant to a participant's plan.
Transport assistance, including wheelchair accessible transport. This supports participants getting to appointments and activities.
Setting up disability support usually starts with a conversation about a participant's current NDIS plan and what kind of support is being sought. Where a support coordinator is involved, they can be part of this initial conversation, though it isn't required.
From there, a support plan is developed following an individual assessment of the participant's goals, routine and any specific needs. Participants are asked to share relevant details from their NDIS plan, so that support can be arranged in line with approved funding categories.
Ongoing support is reviewed periodically and adjusted as circumstances or goals change. There's no obligation to commit to a fixed arrangement before this initial conversation takes place.
Frequently asked questions
What is NDIS disability support?
NDIS disability support covers a range of services funded through a participant's National Disability Insurance Scheme plan. This can include personal care, daily living assistance and support working toward specific goals.
Is a support coordinator required to access disability support?
No, a support coordinator isn't required, though one can be involved where a participant already has one. Participants without a support coordinator can arrange disability support directly.
What does an NDIS plan need to include for disability support to be arranged?
The plan generally needs to show approved funding for the relevant support category being requested. Details of a participant's plan can be discussed as part of the initial conversation about arranging support.
